As a Wellbeing Coordinator, in the recently established Head to Health service in Ipswich, you will collaboratively implement change interventions within a motivational interviewing framework. This encompasses education, skill-building, self-reflection, value-driven decision-making, and fostering community and social connections.
Reporting directly to the Clinical Lead, you will manage the overall implementation of the clients change plan ensuring seamless and timely coordination among internal and external stakeholders, including the client's family or significant others.
