We have a rewarding opportunity at our Yarraville Village Residential Aged Care Home for a parental leave cover Clinical Care Coordinator. This is a full time Monday to Friday 0700-1500 expected to be until June 2025.
The role will be responsible for the delivery of clinical care services including care planning, review and assessments.
What you will bring:
Dedication to resident care and support
Excellent clinical knowledge regarding medical conditions, palliative care, and care of people living with dementia including managing challenging behaviours
High level interpersonal and written skills
Experience managing staff and performance management
Ability to effectively communicate with residents, resident representatives and other health professionals
A good understanding of the aged care reform agenda, including AN-ACC funding and royal commission recommendations related to clinical care
