The Opportunity
IPC Healths Care Finder service supports older vulnerable communities across Melbournes West including Brimbank, Maribyrnong and Hobsons Bay, and Outer West including Wyndham, Melton and Moorabool LGAs. The program aims to provide specialist and intensive assistance through assertive outreach and engagement strategies, to support and assist people to understand and access aged care services and to connect with other relevant supports in the community, including My Aged Care.
