Street Connect is a low barrier, outreach and case management service for young people aged 15 25 years who are street present, homeless or at risk of homelessness. We provide connection, support and referral to young people who are street present in the inner City, and intensive case management and crisis intervention where they are at. We are looking for our next Case Manager to complete our team and continue the professional delivery and development of the Street Connect
