ED Partners Key Responsibilities: Provide compassionate and person-centered care and support to individuals with disabilities. Assist clients with daily living activities such as personal hygiene, meal preparation, and medication administration. Support clients in participating in community activities, social outings, and recreational programs. Collaborate with allied health professionals and support teams to ensure holistic care planning and implementation. Maintain accurate documentation and records of client progress and care activities. Requirements: Certificate III or IV in Disability Care, Aged Care, Individual Support, or a related field (or willingness to obtain). Demonstrated experience in providing care and support to individuals with disabilities or similar vulnerable populations. Excellent communication skills and the ability to build positive relationships with clients, families, and team members. Current driver's license and access to a reliable vehicle (preferred but not mandatory). Valid National Police Check and DCSI Disability-related Clearance (or willingness to obtain).
