About the role An Allied Health Assistant contributes to client care through the provision of clinical support tasks within the multidisciplinary Post Acute Care teams under the direction and supervision of an allied health professional. The position aims to assist in the enhancement of clients' functional capacity and carer needs in order to promote, maintain, and improve their health, functional ability and quality of life. The Service provides a post acute hospital avoidance response to people who are discharged from hospital or who present to emergency departments (and not requiring admission), GP and QAS. Short term multi-disciplinary interventions are provided to address immediate care needs. Services are provided in-home or clinic based. The Post Acute Care Service operates seven days a week including public holidays with extended opening hours – 0730 hours to 1800 hours.
