Working Hours: Full Time / Part Time available

Salary: $75,000 - $100,000 + super + travel allowance

Location: Roxburgh Park, VIC

 

Roles & Responsibilities:

Our Clients vision and mission is centred on delivering exceptional quality care within the Aged Care and Disability Sectors. They provide various community and in-home support services catered towards individual needs. 

 

They are currently seeking a Physiotherapist to service the Melbourne Community through home-care packages.You will be working within a multidisciplinary team to offer the ultimate care to your participants. Providing in-home care and building relationships with participants and their families during a flexible work-week. This role is open to New Graduates as well as Senior Clinicians.

 

As a Physio you are expected to:


Provide high quality Physiotherapy assessments and intervention for consumers within the context of Aged Care in a professional, confidential and culturally safe manner, in keeping with best practice standards
Completing consumer focused Physiotherapy assessments to manage a range of musculoskeletal, neurological, cardio respiratory disorders.
Ensure integrated service delivery and follow up, coordinating and communicating as required
Incorporate a holistic approach to health care that incorporates health promotion activities to consumers according to their priorities and requirements
Ensure information is recorded accurately and in a timely manner in the Management system and that records are maintained in accordance with the Aged Care Quality Standards and company policies and procedures
Ensure privacy and confidentiality is adhered to in relation to consumer’s information
Providing manual handling and education training for carers and family members
Consulting with site staff, doctors, consumer, family and other care professionals to develop consumer care plans, goals, nature of interventions and plan for review
Regularly reviewing consumers outcomes and make any necessary changes to align with consumer goals
Documenting each consumer treatment and general care in clinical notes as per company guidelines
As Attending and participating in meetings, including multidisciplinary, staff and management meetings on site and as directed by your Manager
