As Youth Worker you will focus on the mental health needs of young people within the community experiencing disadvantage.
Your responsibilities will include:
- Building relationships with and support young people at risk of homelessness by facilitating intake and assessment, and developing and implementing innovative strategies.
- Utilising a range of appropriate early intervention strategies to address identified issues.
- Developing and monitoring an individual plan that identifies mutually agreed outcomes, responsibilities & time frames for completion, development opportunities, learning goals, personal goals & career aspirations.
- Providing flexible, timely and individualised short term and long-term case co-ordination and monitoring.
