About the Role Responsibilities include, but are not limited to: Support schools to interpret, develop, refine, and enact departmental policies, practices and procedures relating to students exiting school prior to the completion of Year 12 to ensure they remain in an eligible option. Provide support to schools that will assist in the development and implementation of successful retention and transition strategies and programs that will improve the transition outcomes for ‘at risk' students. Provide appropriate support through the identification of student needs options and intervention to young people and their families to assist in the transition of young people into a viable education, training, or employment. Collate and analyse confidential information, identifying needs and barriers to formulate the basis for the development of a school to post school transition plan for young people (post compulsory participation phase) who leave school before the completion of Year 12 (transition of students in the compulsory participation phase to be determined by region) and provide appropriate case management, advocacy and support as required.
