As an Access & Support Facilitator at EACH, you will:
- Facilitate and support improved access to services for individuals facing barriers
- Provide individualized assessment and care planning aligned with the EACH Consumer Directed Care model
- Deliver high-quality short-term care management and support to address identified needs
- Empower and build confidence in customers and their families/carers regarding service access and utilization
- Collaborate with service providers, leveraging local knowledge to enhance customer access and support
- Advocate for improved service delivery responses to meet the needs of diverse communities
