Your day will be varied and collaborative and will involve integrating and coordinating complex health care services and linkages across general practices with a case load of clients. This role will see you providing individual assessments and tailored care for patients with complex and chronic conditions, and you will deliver high-quality health care, remote and in person patient monitoring and education, as well as a a mix of home visits, working in GP clinics and hybrid work.
You will work with clients to understand their needs and make recommendations for treatments and interventions to encourage them to actively engage in the management of their condition, as well as reducing any barriers that they may be facing to receiving treatments.
You’ll be committed to the principles and practices of community health, primary health and the social model of health, and possess excellent skills in a wide range of clinical assessment, treatment, intervention and client education in chronic disease management. You will also be skilled in motivational interviewing to enhance and facilitate behaviour change towards improved health.
