We are seeking a highly motivated and experienced Intake Officer to join our team. The Intake Officer is responsible for the initial assessment and intake of new participants to our services. This includes gathering information from participants and their families, assessing their needs and eligibility, and developing care plans. The Intake Officer also plays a key role in ensuring compliance with all relevant regulations and standards.
Responsibilities
Conduct initial assessments of new participants to our services.
Gather information from participants and their families.
Assess participants' needs and eligibility.
Develop care plans.
Coordinate the referral of participants to other services as needed.
Ensure compliance with all relevant regulations and standards.
Qualifications and Experience
Registered Nurse or Enrolled Nurse qualification
Experience working with people with disabilities.
Excellent communication and interpersonal skills
Strong organisational and time management skills
Ability to work independently and as part of a team.
