Due to continued growth of services, we are looking for an experienced and motivated Occupational Therapist to join our Home Care Packages Team. This role is responsible for the full range of functions of an Occupational Therapist and works closely with consumers and their families to achieve positive rehabilitative outcomes. In conjunction with the Care Advisors, the occupational therapist will assess consumers’ level of functioning, identify the recovery goals, develop individual goals collaboratively with the consumer and facilitate the provision of a supportive environment where the consumer will have the opportunity of working toward achieving their full potential.

WHAT YOU BRING :


Current registration as an Occupational Therapist with the Australian Health Practitioner Regulation Agency (APHRA) to practice with the National Board.
Member of the Australian Association of Occupational Therapy (Victoria).
Previous experience in occupational therapy assessments and interventions including working with individuals and families.
Experience in assessment and service delivery to aged clients living in the community.
Ability to provide recovery focused interventions to people with complex needs.
Well-developed communication, planning, organisational and time management skills.
Proven ability to establish and maintain effective and cooperative internal and external working relationships.
Strong IT skills and the ability to use a database.
Current drivers’ licence and satisfactory police check.


DESIRABLE:


Knowledge of Home Care Packages and other community services in the Aged Care sector.
Experience with brokerage models of service and Consumer Directed Care.
Knowledge of Carelink Plus.
