As Case Manager your duties will include:
- Utilising your case management skills to undertake initial and ongoing comprehensive assessments and care planning to support customers at home.
- Managing the delivery of services to support the needs identified (psychosocial, medical, physical and vulnerabilities) within the customer’ individualised budget.
- Drawing upon and strengthening relationships with colleagues, services networks and a range of community agencies to support the program and customers.
- Mentoring Support Workers within the HCP team.
On a typical day in this fast paced role, you will visit an older person in their home, follow up on their care requirements; attend a case conference; liaise with our Clinical Case Manager in relation to customer's clinical needs; attend to case management for your portfolio and provide reporting for governance and compliance requirements; complete rostering requests with a focus on customer needs and their financial package etc.
