So what will you be doing in this amazing role?

• You will manage client care needs and complete initial assessments of client needs to determine home care needs using clinical nursing skills.
• Provide client progress reports to relevant stakeholders, such as internal staff, families, carers and external service providers.
• Review and monitor client care notes to ensure services are being delivered effectively and consistently, as well as to identify trends in client care.
• Facilitate the variety of clinical trainings to the community support workers.

What are you going to bring along with you?

• Tertiary qualification in Nursing or related discipline is mandatory.
• Current Nurse registration with AHPRA is mandatory.
• Previous experience in a related role, such as client or case management.
• Ability to problem solve and work effectively in a very fast paced environment with multiple competing priorities.
• Current and active driver’s licence.

Salary - 85k - 90k + Superannuation + Bonus + Car
