The Chronic Disease Registered Nurse Care Coordinator in the Pathways++ program provides tailored, person-centred care for patients with complex and chronic conditions, ensuring EACH delivers exceptional care consistently.

What You’ll Be Doing:

• Conduct comprehensive health assessments and collaborate with clients to understand their conditions, treatments, and active management strategies.
• Identify and prioritise health issues impacting patients.
• Support patients in overcoming barriers to engaging with health professionals or services.
• Assist clients in setting goals and developing care plans to address their health issues.
• Initiate referrals to appropriate health professionals and services, including social prescribing.
• Participate in clinical supervision and engage with key stakeholders.
• Ensure customer-focused, high-quality services that comply with Quality and Compliance standards and best practice care.
• Maintain accurate customer records and complete documentation timely and accurately.
• Adhere to professional and ethical standards.
• Foster a team culture aligned with EACH values and behaviours.
• Contribute to continuous program development and quality improvements.
• Represent the service at internal and external forums, meetings, and networking events.
• Attend performance meetings with relevant funders to discuss performance and compliance when requested.

You will join a close-knit team who work collaboratively to support the business with queries, requests, People & Culture platforms, processes, and policies. In this role you will be responsible for maintaining accurate employee and system records, processing changes relating to the employment lifecycle, and improving the employee experience.

In this role you will: 

• Be the first point of contact for the business for all People and Culture related matters and queries
• Provide support, guidance and coaching to people leaders and employees on People and Culture processes, platforms and the employee lifecycle
• Assist in the initial process of employee concerns or grievances  
• Assist in handling employee disciplinary processes and maintaining relevant documentation
• Identify, undertake and contribute to ongoing continuous improvement initiatives to enhance the employee experience

• Collaborate with or escalate to the relevant area of People and Culture when working through employee matters and queries, when required
• Prepare and manage employment contracts and other letters related to the employee lifecycle

• Ensure data integrity and all employee records are accurate and up to date

About the Role

Djirra will be organising event(s) during our 16 Days of Activism campaign.

Working closely with the Manager, the Event Coordinator will organise, oversee and coordinate the event(s), including, but not limited to:-

• Planning the event from start to finish according to requirements, target audience and objectives
• Developing a sponsorship prospectus, attracting sponsorship and engaging with stakeholders with the purpose of gaining sponsorship
• Lead promotional activities for the event
• Maintain and update event information, including via websites
• Engage and oversee a suitably qualified and experienced event management company, preferably an Aboriginal owned and led business, to support operational delivery in the lead up to and throughout the gathering
• Manage expenses within the budget
• Maintain meeting calendars and ensure event planning is on track
• Ensure the event runs and is completed smoothly, and resolve any problems that might occur.
