Key Responsibility and Duties:
• Working directly with your clients, their family members, and stakeholders to identify participants care needs to achieve their goals and organize care delivery accordingly.
• Create and maintain support/care delivery plan and keep that UpToDate, ensuring the service meets all accreditation standards.
• Create and maintain other necessary forms and documents related to client.
• Communicate and work with stakeholders (support coordinator, allied health practitioner etc) ‚ to ensure right strategy in place to deliver quality care and work towards achieving participants goals.
• Maintain participant's file up to date as per organisational guideline and regulatory bodies guideline.
• Share reports, schedule of support, roster of care, weekly schedule etc with stakeholder in timely manner.
• Managing and investigating incident and accident, feedback and complain.
• Check the timesheet reports/comments daily from deputy and take necessary action if any issues/risk noticed in the report.
• Attending participants phone call and design and resolve participants issue on daily basis.
• Work with other department (for example, Hr and roster team, BDM/engagement team etc) to ensure participant receive right staff with right care.
• Work with support delivery team (Support Staff, AIN, EN, RN) to understand care issues, explain care plan and organise training (with assistance from HR team) wherever needed.
• Work with HR team to organise required support staff as per roster of care/ participants requirement.
• Work with BDM/Engagement Team with onboarding new participant, Budget planning, gap analysis.
• Development, implementation, monitoring, and review of care plans.
• Comprehensive and accurate assessment of participants.
• Assisting with the orientation of new staff and ongoing professional development.
• Prioritizing and managing a high volume of inbound and outbound calls.
• Acting as the primary point of contact for our support staff and clients regarding any incidents and guidelines regarding the service.
• Providing advice and support for any immediate crisis situations and liaise with; clients, emergency services, stakeholders and frontline
workers to achieve optimum outcomes.
• Providing professional direction and support to staff in delivery of care and best practice to clients every week once the roster is
published.
• Ensuring compliance and legislation is adhered to at all times.
• Compile and prepare reports for management and other external stakeholders and communicate timely basis.
• Ensure quality of service (for example: by meeting with participants, actioning and documenting feedbacks and complains, spot check etc.)
• Prompt reply on emails or teams or phone is a must, during office hours as well as outside office hours.
• Must complete daily tasks daily, so time management is very crucial.
• Always check care email and respond to it.
• This job is result and responsibility driven not hour driven, so until you complete your task or fulfill your full responsibility the job is not finished. Your performance will be monitored by the results you achieved.
• Report to Engagement Manger regularly
Qualification:
Tertiary qualification in Business or Accounting or Business management or Marketing or Health care or prior experience in sales and relationship management
Skills and Experience:
• Advanced skills in MS Office suits
• Excellent written and verbal communication skills.
• Good understanding of business process and internal controls related to Health care.
• Experience in monthly and EOY Reporting and Auditing
• Demonstrated experience in sales and management functions.
• Excellent management skills
• Excellent time management skills
• Intention to learn and take initiative
