• Work closely with Senior Managers and Accounts Management team to align marketing activities and materials to mlcoa’s sales and marketing plans
• Responsible for creating and preparing all promotional materials and campaigns alongside driving technical requirements.
• Manage mlcoa LinkedIn pages, including content creation, reporting on results and optimising activity accordingly.
• Plan, create and schedule weekly and monthly communications in line with promotions, events and other marketing initiatives.
• Coordinate marketing calendar to ensure all activity is on track as planned and stakeholders are kept up to date on the status of work.
• Ownership of brand websites ensuring all content is up to date, working closely with external agencies to carry out required maintenance and updates.
• Develop and create all marketing materials in line with brand guidelines including flyers, product sheets, presentations, brochures and infographics.
• Organise and execute conferencing events, sponsorship and association relationships. Including end to end support of quarterly in-person educational seminars.
• Organise and oversee mlcoa’s national webinar schedule, including setting up event in Teams Webinar, preparing marketing material and comms to advertise to customers (Quick word, LinkedIn, mlcoa Website, vault Resources), support speaker with formatting slides and on the day of the session, set up ‘’resources webpage’’ and event survey etc.
• Manage mlcoa team email signatures via Exclaimer Cloud, including producing banners in line with current campaign
• Oversee the mlcoa pages on the MedHealth intranet, including internal and external document management
• Support and collaborate with stakeholders across mlcoa and wider MedHealth organisation.
• Monitor mlcoa national email inbox, and distribute correspondence to the appropriate individual, or state / territory office

This position will be a part of our Social Activity Group with the key focus being to work alongside older people whilst in the supported social activity group to enable their identified needs and goals to be fulfilled, to make decisions and choices about their own lives, to facilitate their access and participation in their community, and to remain living in their homes independently.

Partnering with GP practices and Eastern Melbourne Primary Health Network, this program delivers patient-centred care to:

• Improve health literacy.
• Reduce hospitalisations.
• Connect clients with primary and community health services, ensuring the right care at the right time in the right place.

About the Role

As a Registered Nurse Care Coordinator, you will:

• Deliver tailored, patient-centred care to clients with complex and chronic conditions.
• Conduct comprehensive health assessments and collaborate with clients to understand their health needs and develop personalised care plans.
• Provide chronic disease education, health coaching, and self-management support.
• Monitor for early signs of deterioration and escalate care as needed.
• Foster relationships with external stakeholders, including GP practices and specialist health services, to ensure integrated care.
• Perform home visits and face-to-face consultations to improve medication adherence and reduce hospital admissions.

Key Responsibilities

• Identify and prioritise health issues impacting patients.

• Ability to establish good and respectful working relationships with all Council staff and other.
• Excellent verbal and non-verbal communication skills to empathetically manage customer enquiries and sensitively manage customers displaying challenging behaviours.
