As an AOD Support Worker LLE, you'll provide support in the daily operations of EACH’s AOD Rehabilitation Program, in line with EACH policy and procedures and in conjunction with the MH & AOD Service Stream. This will include delivering within a multi-disciplinary team high-quality services, interventions, and practice expertise to ensure best outcomes for residents.

You will achieve this through your demonstrated experience and expertise in working with residents, bringing your lived or living experience to the role and collaborating with your team in creating a best practice and supportive environment that leads to successful resident outcomes.

What You’ll Be Doing

• Providing one on one and group support to residents from a LLE scope of practice.

• Provide high quality engagement, assessment, treatment, and support to individuals who access the AOD Residential Rehabilitation program. This includes a key worker role for residents.

• Facilitating interim support as required for clients awaiting the commencement of their program.

• Contribute to the provision and review of AOD Residential Rehabilitation treatment plans as part of the team, documenting in the resident’s health record.

• Supporting residents with developing and/or reconnecting and implementing the tools and skills for a successful transition back to the community, including but not limited to assisting with meal planning and preparation, daily chores, and active engagement in the program.

• Provide a number of evidence-based interventions such as psycho education, motivational interviewing, mindfulness, basic CBT etc.

• Dynamically manage risk, inclusive of but not limited to AOD, physical health, mental health and family violence related red flags, involve leadership and following EACH policy and procedures.

• Provide family inclusive practice interventions when indicated, ensuring the client voice is at the forefront of communication and interventions.

• Participate in the orientation and ongoing support of residents.

• Provide practical and emotional support to residents throughout their time, including during admissions and transitions, in conjunction with the multidisciplinary team.

• Demonstrate ability to understand and apply inclusive and intersectional practice when working with people from diverse communities, such as cultures, genders, sexualities, bodies, abilities, spiritualities, ages and backgrounds.

• Deliver much needed Early Childhood (EC) Supports under the NDIS to children aged up to 9 years within schools, homes and the community
• Manage an allocated caseload and provide evidence-based therapeutic services in a timely manner
• Collaborate with our wider multidisciplinary team of Occupational Therapists, Speech Pathologists, Key Workers and Allied Health Assistants

This is a permanent part-time (minimum 3 days/week) position.

About you

• Extensive experience working with children aged from 0 up to 9 years (ideally) and their families
• Experience working as a Key Worker for a registered NDIS provider would be ideal
• Strong knowledge and advocation for the key worker model
• Passionate about providing high quality, evidence-based therapeutic supports to children
• Demonstrated suitability for working in a multidisciplinary team
• Excellent communication skills, coupled with exceptional time management and organisational skills, to manage a caseload
• Genuine passion for working in the disability sector and delivering high quality, evidence based therapeutic ECA supports

The Early Intervention Psychosocial Support Response (EIPSR) program is a joint partnership between Eastern Health and EACH providing both clinical and psychosocial supports to Mental Health Consumers incorporating a holistic strength-based approach enabling Consumers to identify their individual recovery needs.

• Full-time opportunity until 31st march 2025 (possible extension)
• Based in Boronia - Outreach position
• $43 - $47.33ph (depending on experience) + 11% Super + Generous Salary Packaging benefits of up to $18,550    

As a Support Worker, you will be managing a caseload of clients whom you will assist resolve and/or reduce barriers to stable self-care with low intensity community supports or access higher intensity/duration supports to achieve therapeutic outcomes.

Responsibilities of the role: 

• Assist clients (aged between 16 to 65 years) by delivering psychosocial support to work towards identified recovery goals. 
• Improve clients capacity for self-management of their mental illness, & develop the knowledge and confidence they need to make decisions & choices about their own support needs.
• Enhance client coping skills & decrease self-harming behaviour, learn or relearn skills, and develop confidence, required for activities of daily living.
• Improve social and relationships skills & develop or strengthen family and social networks.
• Develop & consolidate client links with educational, vocational training & employment opportunities.
• Assist clients to adopt a healthy lifestyle & minimise/reduce behaviours that are, or likely to be harmful.
• Whilst there may be clients who would like to consider their NDIS possibilities, this role is NOT working within the NDIS.

We are looking for a flexible thinker with not only extensive experience, but also demonstrated skills and knowledge in Mental Health and the Social Model of Health to join our multidisciplinary Mental Health team. 

To be successful in this role, you will have: 

• Understanding of mental services interventions for responding to clients with complexity of needs.
• Understanding of vulnerabilities which people with psychosocial disabilities experience. 
• Ability to collaborate & work in partnership with Eastern Health Case Managers, Team Coordinator, your clients, carers, family and EACH peer workers to assess, design & review client recovery plans.
• Ability to develop & maintain effective relationships within the team & with external stakeholders.
